My read is she only realised in that last scene (on the prison visit) that he did indeed suffer from the personality disorder that she thought he only used in his defence to get a manslaughter verdict instead of murder. He actually thought she really did mean it when she asked him to "smash the face in" of the rapist, in a jokey sort of way when they were in bed. That's how I read it too; although we discussed the possibility that she might have realised he was a butter and utilised it for her benefit.
