    I've filled it in, but as someone who derives more than 50% of his income from gambling alone, the questions are a little vague and don't allow for more than one repsonse.

    Done but would agree with Traf that there needs to be the option for multiple answers, for example i use a computer, a tablet, and a smart phone to place bets
