I had a chat recently about this: I'd like to see more docs (you'd ones in particular) available in a&e for example, so that waiting times are much lower. Utilising any 'slack' times for research/reading to help with career development and specialisation later. I do understand that has cost implications though. The nhs has become a victim of its own success, and sooner or later we more have to confront some truths, and decide if we want a higher tax economy, or a medical service that is excellent but slightly streamlined in what it offers.
